How the reconstitution math actually works

Every number this calculator returns comes from three inputs: the peptide mass in the vial (in milligrams), the volume of bacteriostatic water added to it (in milliliters), and the weekly dose a clinician has set. The first two give a concentration, and concentration is the bridge between a dose written in milligrams and a mark you can read on a syringe.

Concentration equals peptide mass divided by water volume. A 10mg tirzepatide vial reconstituted with 1mL of bacteriostatic water is 10mg per mL, the same as 10,000mcg per mL. Add 2mL to the identical vial and it becomes 5mg per mL. The powder never changed. Only the water changed how far each milligram is spread, and therefore how many marks it occupies.

To turn a dose into a syringe reading, divide the dose by the concentration to get a volume, then convert that volume to insulin units. A U-100 syringe is marked so 100 units equals 1mL, which makes one unit 0.01mL.

Worked example, starting dose: - Vial 10mg, water 1mL, concentration 10mg/mL. - Dose 2.5mg. - Volume equals 2.5mg divided by 10mg per mL, which is 0.25mL. - Units equal 0.25mL times 100 units per mL, which is 25 units.

Because 10mg/mL is a round number, the titration ladder lands on clean marks: 2.5mg is 25 units, 5mg is 50 units, 7.5mg is 75 units, and 10mg fills the barrel at 100 units. That also reveals a ceiling worth planning for. One U-100 syringe holds at most 100 units, so a 12.5mg dose (125 units) or a 15mg dose (150 units) at this concentration will not fit in a single draw. Reaching the top of the ladder means either splitting the volume or reconstituting more concentrated so each milligram occupies fewer marks. The calculator does that arithmetic for whatever vial and water you enter.

Why the exact unit mark matters, and the mistakes that break it

Tirzepatide is potent by volume, so a small measuring error becomes a meaningful dosing error. At 10mg/mL, five stray units is 0.05mL, which is 0.5mg, a full 20 percent of the 2.5mg starting step. The most common errors are conversion mistakes, not technique:

- Confusing milligrams and micrograms. 2.5mg is 2,500mcg. A misplaced decimal moves the dose by a factor of a thousand. - Reading units as if they were a mass. Units are volume marks worth 0.01mL each, not milligrams and not micrograms. - Using a U-40 syringe against a U-100 scale, or a tuberculin syringe marked only in mL. Unit marks do not transfer between syringe types. - Forgetting to recalculate after a switch to a different vial size or water volume. A reading that was correct last month is wrong the instant the concentration changes. - Trapped air displacing solution, which quietly reduces the volume actually drawn.

The safeguard is simple: re-derive the unit mark every time vial size or water volume changes, and confirm the syringe is genuinely U-100 before trusting any number.

Why a five-day half-life sets the schedule

Tirzepatide has an elimination half-life of roughly five days, and that one number explains both the weekly injection interval and the four-week spacing between titration steps. A five-day half-life means plasma levels fall slowly enough that once-weekly dosing holds fairly steady exposure between doses, with only a modest peak-to-trough swing. It also means the drug needs about five half-lives, close to four weeks, to reach steady state at any given dose, which matches the roughly four-week figure in the prescribing information.

That steady-state timeline is why each step is held for four weeks rather than advanced on feel. Moving up before the current dose has plateaued stacks a rising level on an incompletely accumulated one, the setup for the nausea, vomiting, and early satiety that drive people off the compound. Holding each step lets both blood level and gut tolerance settle before the next increase.

The stepwise 2.5mg escalation, and why the start is not the target

The labeled schedule opens at 2.5mg once weekly for four weeks, moves to 5mg, then may rise in 2.5mg increments no more often than every four weeks toward a 15mg maximum. The 2.5mg start is deliberately sub-therapeutic. Its job is tolerance, not a glucose or weight effect. Meaningful response in the trials clustered at the 5mg, 10mg, and 15mg maintenance doses.

That structure came out of the SURPASS program in type 2 diabetes and the SURMOUNT program in obesity. In SURMOUNT-1, the 15mg arm produced about 21 percent average body-weight reduction over 72 weeks, reaching roughly 22.5 percent among participants who stayed on treatment, with the 5mg and 10mg arms lower. That gradient is why 2.5mg is treated as a ramp and not a destination. Where a person settles on the ladder is a clinical decision weighed against response and side effects, and it belongs with the prescriber, not the syringe.

Biomarkers worth trending alongside a tirzepatide protocol

Tracking turns a dose into evidence over time. The markers worth trending split into efficacy and safety, and most are inexpensive to repeat:

- HbA1c and fasting glucose. The core glycemic readout, which tended to improve over 8 to 12 weeks at working doses in the trials. - ApoB and triglycerides. The atherogenic lipid picture generally moved favorably as weight and insulin resistance fell. - Fasting insulin and HOMA-IR. As insulin resistance improves, these tend to fall. In the SURPASS-2 analysis, fasting insulin dropped roughly 15 percent at 10mg and 21 percent at 15mg, with HOMA2-IR down about 20 to 24 percent, a signal the compound is changing metabolic function and not only lowering weight. - Body weight. The most visible endpoint, best logged on a consistent schedule under consistent conditions. - Liver enzymes (ALT, AST). These often improve as liver fat drops, but a baseline and periodic recheck give context. - Resting heart rate and blood pressure. GLP-1 class agents can nudge heart rate upward, so both are reasonable to log.

A baseline before starting and a recheck near three months give every later change a reference point. Interpreting those numbers, and any decision to adjust a protocol, belongs with a licensed clinician.

Why a reconstituted vial can drift from the number on the screen

The arithmetic only holds while the vial actually contains what its label claims and stays stable after mixing. A few well-documented factors can quietly break that assumption, and each is a reason the reading belongs with the preparation a clinician set up and the instructions on the actual label:

- The diluent matters. Bacteriostatic water carries a benzyl alcohol preservative, which is what a labeled multi-withdrawal window assumes. Plain sterile water has none, so a preparation meant to be drawn more than once depends on which diluent was used. - Peptides are fragile. Foaming, hard agitation, heat, and light can degrade peptide and cost potency that no calculator can see, which is why handling and storage conditions appear on the label. - Cold, dark, and never frozen. Reconstituted vials are typically kept refrigerated at 2 to 8 degrees Celsius and out of light, and freezing damages the peptide. - Beyond-use dates are real. A bacteriostatic-water preparation commonly carries a beyond-use window on the order of 28 days, and cloudiness or visible particulates mean the solution is no longer what the math assumed. - Concentration changes silently. A different vial fill or a different water volume changes the concentration and every reading with it, so a unit mark that was correct before is wrong the instant either changes.

None of this is a prescription, and none of it replaces the instructions on an actual label or the judgment of the clinician who set the dose. It is the context that decides whether the number on the syringe still means what the calculator says it means.
