The Mechanism, Briefly
Growth hormone promotes sodium and water retention. That is well established, and it is the same effect behind the puffiness, ring tightness and morning facial fullness people report on GH peptides and on growth hormone itself.
Most of the body absorbs a bit of extra fluid without complaint. The carpal tunnel does not. It is a narrow, rigid passage at the wrist through which the median nerve runs alongside nine tendons, bounded by bone on three sides and a tough ligament on the fourth. There is no room to expand. A small increase in fluid volume inside that space raises pressure on the nerve, and the nerve reports it as numbness, tingling, or a burning ache.
That is why the pattern is so specific: thumb, index, middle finger, and the thumb side of the ring finger, which is the median nerve's territory. The little finger is usually spared, because it is served by a different nerve that does not pass through the tunnel.
Worse at night and on waking is the classic timing. Fluid redistributes when you lie down, and many people sleep with wrists flexed, which narrows the tunnel further.
Carpal tunnel syndrome is a recognized effect of growth hormone excess, which is also why it appears in the clinical picture of acromegaly. Which fingers. Median nerve distribution, meaning thumb through the thumb side of the ring finger, points toward the carpal tunnel. Numbness in the little finger, or a whole-hand or whole-arm pattern, points somewhere else and is worth flagging sooner.
Time of day and duration. Night and early morning only, resolving within minutes of shaking the hand out, is the typical presentation. Constant numbness is a different situation.
Onset date against dose changes. This is the single most useful field. If the symptom started nine days after a dose increase, that is a real signal. Without the dates it is a guess.
Severity, on any consistent scale. One to five is fine. What matters is that it is the same scale each time, so the trend is readable.
Grip strength, or dropping things. Weakness and clumsiness are a different order of symptom than tingling and should move the timeline for seeing someone forward.
Ring and shoe fit, morning weight. These track the general fluid picture and give context. If the hands, the rings and the scale all moved together in the same week, that is one story rather than three.
Markers Worth Having in the Picture
IGF-1, drawn fasted and at a consistent interval from your last dose, using the same lab each time. It is the practical gauge of how hard the GH axis is being pushed, and fluid effects generally track with how far it has moved.
Sodium and potassium, on a standard metabolic panel, since the mechanism is a sodium and water effect.
Fasting glucose and fasting insulin. Growth hormone is counter-regulatory to insulin, and this is worth watching on any GH-directed protocol regardless of symptoms.
Thyroid markers, if your provider is looking. Hypothyroidism independently causes fluid retention and carpal tunnel symptoms, and it is one of the things a clinician will want to rule in or out rather than attributing everything to the protocol.
Red Flags: Stop Tracking and Get Seen
Take these to a clinician promptly rather than logging them:
Persistent median nerve compression can cause lasting damage. The reason to keep a dated log is so that the person you see can tell quickly whether this is a fluid effect that will settle or something that needs imaging and nerve testing.
